Your account setup depends upon receipt of this completed form along with your State Resale Tax Exemption Certificate or the Multijurisdiction Uniform Sales & Use Tax Resale Certificate. Download Blank Multijurisdiction Uniform Sales & Use Tax Resale Certificate from the mtc.gov site

For customers paying via credit card, download the Blank Credit Card Authorization Form and email it to [email protected]. If unable to email the form, please email [email protected] the best contact for us to reach out to for credit card information via phone. Credit cards provided will be kept on file for future orders (an additional 3% processing fee and a 35% authorization hold on the total order amount; the card is charged after the order ships). Any companies with multiple cards are advised to let us know in advance the last four numbers of the card they prefer to use to prevent delays in the order entry and production process.
